Are you aware you can book an appointment with our Mental Health Specialist without seeing a GP first?

This service is available for patients aged 16-64 who are not currently being seen by NHS Mental Health Services. If you are worried about your mental health, feeling overwhelmed or low and needing to talk to someone about seeking relevant support please ask the receptionist for an appointment with our mental health specialist.

If you would like to register with our practice you must live within the practice catchment area.

You will be asked to complete a registration form and health questionnaire.

Following registration it may take some time for your medical records to arrive from your previous GP.  To ensure continuity of care, you may be offered an appointment with our practice nurse who will take details of your past medical history, any medication you are taking and carry out a simple health check.

You may print off a registration form and health questionnaire using the links below.  Please complete the forms and bring them with you on your first visit to the practice. Please also bring photographic identity and a utility bill showing your current address. If you are taking regular repeat medication please bring your re-order slip with you.
